New Patient Information
Dr. King and staff would like to welcome you to their
office. We are honored you have chosen us for your medical care. We,
at King Family Medicine, pride ourselves on a unique combination of
Caring, Compassion, Expertise and Technology in providing health care.
The following information should guide your expectations related to
services you may receive.
As a new patient you will be required to provide us
with some general information; such as, address, insurance, health history,
etc. We have made these forms (see below) available on our website for
your convenience. If you are unable to download these forms and complete
them prior to your appointment, you will need to arrive 30 minutes prior
to your scheduled appointment time.
Office Hours
Our business hours are from 7:45 a.m. to 4:30 p.m. Monday through Friday.
We are closed from 12:00 – 1:15 each day for lunch. Our answering
service will assist you if you need to reach us beyond our regular business
hours.

Preventive Exams
The preventive examination is one of the most important medical visits.
This is an opportunity to update your health history and testing. This
is also a time to assess your risk factors for major health concerns.
There are several standard tests that occur at your preventive exam.
Notification of Test Results
Generally, lab results are forwarded to our office within 24 hours of
your visit. Our office will notify you of any abnormal findings. If
you have not been notified of your test results within 3 business days,
please call our office at 340-2600.
Prescription Refills
Please contact your pharmacy first for refill requests. We will make
every effort to respond to refill requests within 24-hours. To avoid
a lapse in your medications, it is a good rule of thumb to notify the
clinic when you have a one week supply left of your medication(s).
Insurance
Dr. Steve King participates in many insurance plans. We strongly encourage
our patients to contact their insurance company and verify their benefits,
co-pays and deductibles prior to their appointment.
Any co-pay, deductible, or non-covered services are
the patient's responsibility. Co-pays and deductibles are expected at
the time of service. Paying your co-pay or deductible at the time of
your visit will help minimize the number of invoices you receive from
our office.
**Please bring your insurance card and driver’s
license to your appointment.
Payments
King Family Medicine accepts; Cash, Check, Visa, MasterCard, and Debit
cards for payment of any account. Payments are accepted over the phone,
in person or by mail.
FORMS for New Patients
These forms require the Adobe Acrobat Reader. If you need the Reader,
you can download
it free to your computer. We encourage our patients to download the
forms below and complete them prior to their appointment.
